1. Who we are
Relyfe is a telehealth and wellness platform operated by Quantum Recovery LLC ("Relyfe," "we," "us"). Relyfe is a technology and brand platform: medical care is provided by independent, licensed healthcare providers, and medications are dispensed by independent licensed pharmacies. This Privacy Policy describes how we collect, use, and share information when you use ReLyfe.health, our waitlist, our member portal, and related services (together, the "Services").
2. Information we collect
Information you provide
- Contact and account information: name, email, phone number, mailing address, date of birth, and login credentials.
- Health information: health history, symptoms, medications, lab results, and other information you submit through intake forms, assessments, messages with your care team, or visits — as needed to facilitate your care.
- Payment information: processed by third-party payment processors. We do not store full card numbers on our systems.
- Communications: messages, emails, and support requests you send us.
Information collected automatically
- Device and usage data such as IP address, browser type, pages viewed, and referral source.
- Cookies and similar technologies used for site functionality, analytics, and security. You can control cookies through your browser settings.
3. How we use your information
- To operate the Services, create your account, and manage your membership and billing.
- To connect you with licensed providers, pharmacies, and laboratories, and to facilitate scheduling, messaging, prescriptions, fulfillment, and shipping.
- To communicate with you about your care plan, orders, account, and the Services.
- To improve the Services, maintain security, prevent fraud, and comply with legal obligations.
- With your consent, to send marketing communications. You can opt out at any time.
4. Health information and HIPAA
Health information you share in connection with care may be protected health information ("PHI") under the Health Insurance Portability and Accountability Act ("HIPAA") when held by covered entities such as the licensed providers, pharmacies, and laboratories involved in your care. Those entities maintain their own privacy practices and notices. Where Relyfe handles PHI on behalf of a covered entity, it does so under business associate agreements and uses and discloses that information only as permitted by HIPAA and applicable law.
We use administrative, technical, and physical safeguards designed to protect your information, including encryption in transit, access controls, and multi-factor authentication on systems that handle sensitive data.
5. How we share information
- Your care network: licensed providers, pharmacies, and labs involved in your care.
- Service providers: vendors that power the platform — such as telehealth infrastructure, hosting, payment processing, email delivery, analytics, and customer support — bound by contractual confidentiality and data-protection obligations.
- Legal and safety: when required by law, legal process, or to protect the rights, safety, or property of you, us, or others.
- Business transfers: in connection with a merger, acquisition, or sale of assets, subject to this Policy's protections.
We do not sell your personal information, and we do not share your health information for third-party advertising.
6. Your privacy rights
Depending on where you live, you may have rights to access, correct, delete, or receive a copy of your personal information, and to opt out of certain processing.
California residents
Under the California Consumer Privacy Act (CCPA/CPRA), California residents may request: (a) disclosure of the categories and specific pieces of personal information we collect; (b) deletion of personal information, subject to legal exceptions; (c) correction of inaccurate information; and (d) to opt out of "selling" or "sharing" as defined by law — noting that we do not sell personal information. We will not discriminate against you for exercising these rights. To exercise any right, email info@ReLyfe.health with the subject line "Privacy Request." We may need to verify your identity before acting on a request.
7. Data retention
We retain personal information for as long as needed to provide the Services, meet legal, regulatory, and recordkeeping obligations (including medical-record retention requirements that apply to providers), resolve disputes, and enforce agreements. When information is no longer needed, we delete or de-identify it.
8. Age requirement
The Services are intended for adults 18 and older. We do not knowingly collect personal information from anyone under 18. If you believe a minor has provided us information, contact us and we will delete it.
9. Changes to this policy
We may update this Privacy Policy from time to time. We will post the updated version with a revised "Last updated" date, and where required by law, provide additional notice. Your continued use of the Services after an update means you accept the revised Policy.
